China's first application of computer three-dimensional surgery decision system to separate children

Release date: 2016-01-14

Shanghai Fudan University Affiliated Pediatric Hospital revealed on the 12th that the experts of the hospital used the "Computer-Assisted Liver Three-Dimensional Surgery Decision System" (CAS) for the first time to successfully "divide" a one-on-three-month-old conjoined brother. The hospital revealed that the operation started in the morning lasted more than five hours and ended smoothly.

(Doctors use CAS to grasp the anatomy through a 3D rotating picture.)

(The doctor successfully separated the liver for more than an hour during intense surgery.)

It is reported that this is the first time that China has applied the CAS system to the separation of infants.

Zheng Shan, deputy dean of the Pediatric Hospital affiliated to Fudan University and a specialist in pediatric surgery, said that the difficulty of the operation lies in the fact that the two brothers have abnormal intrahepatic blood vessels in addition to the liver parenchyma. Doctors use CAS to use hybrid imaging and segmentation technology to clearly reproduce the three-dimensional situation of three different sets of blood vessels in the liver of two people, and to accurately separate the connected liver from different angles, avoiding vascular damage and Bleeding. According to the hospital, the intraoperative blood loss was only 30 ml.

Dong Xiaoran, deputy director of Pediatric Surgery, also said that human soft tissue changes greatly during surgical tumbling. If there is any doubt during surgery, doctors can grasp the anatomical structure through 3D rotating picture, which is beneficial to reduce bleeding and precise separation during operation.

It is reported that the mother of the couple brothers was from Jiangxi, and was diagnosed with a malformation at 31 weeks of pregnancy. At the end of September last year, the two brothers were immediately taken to the NICU ward of the Pediatric Hospital. The doctor found that the child had a chest and abdomen joint, up to the xiphoid fusion, the sternum stem separated, down to the umbilical cord. The two share an umbilical cord. The doctor decided to split the brothers for 3 months and the overall weight of about 10kg.

After the two children were admitted to the hospital, the Pediatric Hospital General Surgery, Plastic Surgery and other teams conducted several discussions. The team led by pediatric surgeons Zheng Shan and Dong Yiran decided to perform chest and abdomen separation surgery on the couplet on the 12th. . At 10:20 on the same day, the operation began, and at about noon, the liver was successfully separated.

Conjoined babies are a rare clinical congenital malformation. 40%-60% of the children will die in the uterus, 35% will die within one day after birth, and only about 30% of the conjoined babies may survive after birth, and female More infants. In 2000, Shanghai Fudan University Pediatric Hospital successfully separated rare joint babies; in 2015, the hospital used 3D printing technology for the first time to accurately assist in the separation of conjoined babies.
