Development plays an important role in regulation.
Obesity can also hurt the liver? This is beyond the expectations of many people, and it is an indisputable fact in the eyes of experts. Recently, the research team led by Chen Qi, director of the Key Laboratory of Molecular Pharmacology of Cardiovascular Diseases of Jiangsu Province, and the Director of the Collaborative Innovation Center for Cardiovascular Diseases Translational Medicine of Jiangsu Province, found that a type of scavenger on the surface of macrophages The protein receptor of the receptor (SR-A1) plays an important regulatory role in the development of metabolic inflammation such as nonalcoholic fatty liver (hereinafter referred to as NAFLD).
At present, the global incidence of NAFLD has increased significantly in recent years. The prevalence rate in China is between 15% and 17%, which has greatly exceeded the incidence of 9.7% of type 2 diabetes. With the increase of age, the prevalence of fatty liver increases year by year. The incidence of fatty liver in obese people is as high as 40%, and mainly NAFLD, accounting for about 90% of fatty liver.
To this end, Chinese experts are looking for the pathogenesis of metabolic diseases.
Nearly 300 million patients with lipid metabolic disorders in China
In the past 30 years, obesity has been a global trend. About 1.46 billion adults in the world are overweight, of which 205 million men and 297 million women are obese. At present, obesity is one of the three risk factors affecting human health along with smoking and drinking.
"Atherosclerosis, type 2 diabetes, nonalcoholic fatty liver and obesity are called metabolic diseases. They are 'four melons on a vine'." Chen Qi told reporters that according to the latest epidemiological survey. It is estimated that there are nearly 300 million patients with various lipid metabolism disorders in the Chinese population.
From a medical point of view, NAFLD is caused by excessive accumulation of neutral fat (mainly triglycerides and fatty acids) in liver cells due to various factors such as lipid metabolism disorder. A variety of important tissue damage caused by lipid metabolism disorders, especially cardiovascular and hepatic damage, poses a great threat to health. Therefore, its mechanism and prevention strategy research has always been the frontier of international medical research.
Experts told reporters that although diet control and lifestyle changes have significant improvement effects on hyperlipidemia and fatty liver, they are difficult to implement in real life, and patients are often unable to perform according to their doctor's advice due to various factors. So far, humans have not developed an effective drug to completely cure and prevent liver lipid metabolism disorders.
Obesity can lead to metabolic inflammation
The traditional view is that the occurrence of metabolic diseases such as nonalcoholic fatty liver is mainly induced by metabolites such as lipids accumulated in the body.
However, with the deepening of scientific research, more and more evidence indicates that there is a close relationship between metabolism and inflammation, and obesity and lipid metabolism disorders directly lead to chronic inflammation. Thus, the concept of "metabolic inflammation" has been proposed in recent years in the world.
Is it the same thing about metabolic inflammation and acute inflammation such as redness, heat pain, and bacterial infection? How do we understand metabolic inflammation? What is the relationship between metabolic inflammation and NAFLD?
"Metabolic inflammation is not an acute inflammation that people often say. It is triggered by nutrients and excess metabolism." Chen Qi told reporters.
"Due to changes in modern living habits and the environment, the body's metabolic disorders and the production of some harmful metabolites will cause changes in macrophages in the body and induce chronic low-grade inflammation, called metabolic inflammation. With this 'vine', It can damage human tissues and organs and lead to the appearance of 'melon', which is the occurrence of metabolic diseases."
After all kinds of nutrients are ingested into the body, they affect the metabolic and inflammatory processes in the body through different nutrient receptors. Among them, hyperlipidemia (especially hyper-fatty acidemia) plays a very important role in the occurrence of metabolic inflammation.
Macrophages play an important role
Macrophages in the immune system play an important role in this process. Studies have shown that macrophages are the signal centers of the metabolic inflammatory response, which can regulate the inflammatory response through changes in their own quantitative and polar differentiation.
A large number of human and animal experiments have confirmed that there are a large number of macrophage infiltration in adipose tissue during obesity. About 40% of the adipose tissue in obesity is macrophages. These macrophages are inflammatory genes in adipose tissue. The expression also plays an important regulatory role.
The research team of Nanjing Medical University led by Chen Qi found that there is a protein receptor called SR-A1 on the surface of macrophage membrane, which plays an important role in the regulation of macrophage function. It has been experimentally proven that when its expression is absent, it can lead to a marked increase in inflammation, thereby accelerating the progression of metabolic diseases and even accelerating the development of tumors and vascular lesions; on the contrary, in the case of SR-A1 activation, the level of metabolic inflammation is markedly Reduce, thereby lowering blood sugar, blood lipid levels, alleviating the progression of metabolic diseases, and have a certain therapeutic effect.
Experts say the study addresses the pathogenesis of these diseases from a new perspective and provides new ideas for treatment. At present, some of the results have been published in publications such as Diabetes and Am J Physiol.
How to regulate the body's immune system
Knowing the pathogenesis of metabolic diseases such as NAFLD, how do we regulate this response of the human immune system to keep them at a normal level?
In order to answer this series of questions, the Ministry of Science and Technology set up the 973 program "the mechanism of lipid metabolism disorder leading to the occurrence and development of fatty liver." The project uses a large-scale population cohort to screen, identify and evaluate new biomarkers of NAFLD and the inheritance of hyperlipidemia-related metabolic diseases using modern biological methods such as genomics, proteomics and metabolomics. And serological indicators, the establishment of a non-invasive diagnostic system for NAFLD suitable for Chinese residents, help to discover new pathogenic factors or protective factors of NAFLD or hyperlipidemia, or propose a new cause hypothesis.
According to experts, the research team has established 10,000 people in the general population of 30-65 years old in Guangdong (about 4,700 people with hyperlipidemia, about 2,500 NAFLD); 15,000 cases in Shanghai, up to 15 A follow-up cohort of the year; a pool of 29,365 blood samples from Beijing population and a library of liver biopsy specimens including more than 2,000 patients established through the China Fatty Liver Care Network were established. At the same time, the "lipid metabolism model animal resource library" was also established, and experimental animals were used for related technical research.
The team led by Chen Qi screened and applied the SR-A1 ligands such as dexamethasone and salt algae polysaccharides in a specific way. It was found that both substances can regulate macrophages through interaction with SR-A1. Cell function further plays a role in inhibiting chronic low-intensity inflammation and protecting the body's metabolic diseases. This finding provides a new target for the intervention of metabolic inflammation, immunotherapy for inflammatory diseases. Next, scientists will further study the intrinsic link between lipid metabolism disorders and metabolic inflammation, and how the two synergistically induce and aggravate the molecular mechanisms of fatty liver.
